AHRQ coordinates the development of Common Formats for reporting patient safety data. This activity is authorized by the Patient Safety and Quality Improvement Act of 2005 (Patient Safety Act) and the Patient Safety and Quality Improvement Final Rule (Patient Safety Rule). In collaboration with the Patient Safety Work Group (PSWG), AHRQ has developed Common Formats for three settings of care - hospitals, community pharmacies and nursing homes.
To learn more about Common Formats, please visit AHRQ Common Formats.
The Patient Safety Organization Privacy Protection Center (PSOPPC) provides reporting solutions at the organizational level as well as at the data management level. These solutions create opportunities to develop and validate patient safety data submissions to the PSOPPC and to validate the PSO Profile information.
