The Common Formats for Event Reporting - Diagnostic Safety Version 1.0 (CFER-DS V1.0) are designed for use
across all healthcare settings and specialties. Widespread use of the CFER-DS and reporting to the PSOPPC/NPSD will make it possible to collect, aggregate and analyze diagnostic safety-related information nationally to accelerate learning and improvement.

Event Description
The Event Description contains the complete set of CFER-DS data elements and substantive corresponding answer values that can be used to describe a single Diagnostic Safety Event in outline format.

Form
The Form contains the complete set of CFER-DS data elements and corresponding answer values that can be used to describe a single Diagnostic Safety Event in a question-and-answer format. Note that the question numbers in the Form do not correspond to the item numbers used in the Event Description.

Users' Guide and Glossary
The Users' Guide provides a general overview of the AHRQ Common Formats for Event Reporting as well as specific information about the CFER-DS V1.0: an overview of the conceptual framework, definitions, and helpful information for implementation. It includes clinical examples that illustrate how to apply the CFER-DS concepts and definitions to a Diagnostic Safety Event as well as a glossary of terms used in the CFER-DS V1.0.

Preliminary Report
The "Preliminary Report about a Diagnostic Safety Event for Front Line Clinician Use" is an optional supporting resource that may be adapted and integrated with existing internal event reporting and analysis workflows. Such a resource may facilitate capture of brief information by Clinicians at the time a Diagnostic Safety Event is recognized for later collection of the full set of CFER-DS data elements.
