AHRQ coordinates the development of Common Formats for reporting patient safety events to Patient Safety Organizations (PSOs). This activity is authorized by the Patient Safety and Quality Improvement Act of 2005 (Patient Safety Act) and the Patient Safety and Quality Improvement Final Rule (Patient Safety Rule). In collaboration with the Patient Safety Work Group (PSWG), AHRQ has developed Common Formats for two settings of care - acute care hospitals and skilled nursing facilities.
Active for Reporting
Common Formats that are active for reporting are available for implementation and use by healthcare providers and Patient Safety Organization (PSOs). These versions of the Common Formats are also accepted by the PSOPPC.
Comments can be submitted on all active reporting versions through the NQF Web Site.
Open for Public Review/Comment
Common Formats that are open for public review/comment are published in order to receive feedback from the public prior to becoming active. Comments can be submitted through the NQF Web Site.
Retired (Reference Only)
Common Formats that are retired are no longer active for reporting and should be utilized for reference only. These Common Formats should not be used by healthcare providers or Patient Safety Organizations (PSOs). Retired versions of the Common Formats can be obtained by contacting the PPC Help Desk.
